Webinar:Population Health in the context of the Affordable Care Act: The role and accountability of Hospitals and Health Systems

Non-profit hospitals have historically played an important role in the assessment, design and implementation of health promotion, training, and access to care programs to address both the medical and non-medical needs of the broader community. The planning processes hospitals undertake directly influence how hospitals decide to invest millions of dollars annually in the communities they serve. Hospitals and health systems were uniquely positioned to re-evaluate their planning processes and investment opportunities as a result of the Affordable Care Act (ACA) mandate for completion of a Community Health Needs Assessment and Implementation Strategy. During this webinar, we will provide an overview of the strategic processes, tools, and frameworks used by one health system to guide its 35 hospitals in the strategic planning process. Many of the hospitals worked in collaboration with local health departments and other key community stakeholders, and used a set of tools including evidence based snapshots, to develop high impact strategies. The lessons learned and future challenges and opportunities for engagement of hospitals in the population health discourse will be discussed.
